Nurse Manager - Clare Housing


The Nurse manager is responsible for management and oversight of Clare Housing’s homecare program, which provides care to people living with HIV and other chronic conditions at Clare Housing apartment buildings. This position works closely with the Supportive Housing program, a team of dedicated housing staff, to ensure compliance with meeting MDH regulations and requirements for the Comprehensive Homecare license. The Nurse Manager directly supervises registered nurses and provides work direction to unlicensed resident assistants.


Through the work of the Nurse Manager, people living with HIV in Clare Housing’s homecare program are able to stay in their homes, achieve positive health stability, and reach other significant goals unique to each individual. 

Core Job Functions:



  • Set hours and determine nurse staff needs at homecare sites

  • Meet regularly with nursing staff one-on-one for supervision and provide support onsite throughout each week

  • Cover nursing shifts as needed



  • Complete nursing assessment of all new Home Care Services candidates; implement service agreement with residents; assure HCS residents meet the admission and continuing stay criteria

  • Work with site manager housing staff and site-based nurses to determine initial and annual CADI rate sheet submissions

  • Schedule and meet with resident assistant staff every 90 days to assess skills, provide feedback and support

  • Support the site nurses to complete all 90 day and annual client assessments for homecare residents 

  • Provide clear direction in determining and assessing level of care for homecare clients who are critically ill, discharging out of hospital and TCU settings or in need to transition to a higher level of care

Maintain Compliance 

  • Manage compliance requirements of comprehensive homecare license including nursing policy and procedures, PCC documentation, and completion of Educare modules


  • Provide nursing specific training for all new resident assistants; and ongoing training and education on new devices per homecare clients plan of care

  • Provide training to new nursing staff

  • Lead agency wide training as needed


Care Coordination


Participate in weekly homecare huddles for all homecare locations, along with property management, and Clare Housing staff meetings

Position Requirements:

Education and Experience:

  • One year of work experience as a Home Care Service nurse and/or public health nurse. 

  • At least one- two years of nurse management experience 


Required Knowledge and Skills

  • Knowledgeable of HIV/AIDS disease and medications

  • Have a sensitivity and aptitude for working with individuals who were formally homeless and persons living with HIV/AIDS, mental illness and chemical health problems

  • Respect and communicate effectively with multi-cultural, diverse populations including during times that are stressful and/or emotional

  • Read, write and speak English sufficiently in order to communicate pertinent medical and social service information to residents and to communicate with management and staff

  • Work cooperative with an interdisciplinary team

  • Work independently, exercise good judgment and pay close attention to detail

  • Agile, able to move quickly and easily throughout the workday and throughout the building

  • Excellent interpersonal communication skills; verbal and written

  • Maintain confidentiality of client information

  • Ability to maintain professional boundaries and practice self-care in a position that is confronted often with physical illness, mental illness and chemical use issues

  • Ability to instruct/supervise staff on medication administration and procedures

Licenses and/or Certifications:

  • Registered Nurse currently licensed with the State of Minnesota, BSN education preferred

  • CPR/First Aid Certification required; training provided to obtain certification

  • Driver’s License, State ID or Passport 

Working Conditions and Environment:

This position is located in the supportive housing buildings for adults living with HIV/AIDS and other disabilities. Many of the residents are formerly homeless individuals. The buildings are accessible and smoke free. This position works during normal business hours which may include evening and weekend hours. The person in this position will be exposed to body fluids, infections, odors.
